The same has happened to me when feeding PolyFlex into the V4 filament drive. My guess is also that it is a matter of speed. I wonder how it could happen to you during printing though? Did the filament break first or was it a sling of filament which was sucked into the gear?

I don't regret switching to the octoprint setup. As the raspi is connected to the printer by USB, printing is not affected by a loss of network connection. I like that I can use my smartphone or tablet to control the M2 now (there is no app though - just using the default browser). As my desktop PC ...

However, as the wiring is supposed to run between the filament drives and the extruder motors, a hot-end swap would require taking the filament drives off. Is it OK for any of the V4 guys to post pictures of their hot ends or is the design still proprietary? I still have a V3A on mine and am looking into the 24V upgrade. Not sure if I just get the V3B or go with the V4. Since I primarily print PLA, maybe stick with the V3B and wait for the V4 teflon? ...
