Does Z-offset setting affect every layer, or only the first layer?

All of them. Indirectly.
Reason being, every subsequent layer builds off relative to the first, so that offset, which affects the first layer, actually has an impact on all of them.

I think I've got it.
So, if i"m printing at 0.2mm layer height, I could squish first layer to 0.15mm, and all other layers will be at 0.2 mm, and total model height will be 0.05mm short.
Correct?

More like:
The nozzle starts extruding the squished first layer from 0.15 mm away from the bed. Then it moves up 0.20 mm from
that position for the second layer, and up another 0.20 mm from that position for the next layer, etc......
But yeah, same effect.
