Filament Extrusion Problem

So I've had my printer for a bout a year and have printing ok with it. (still have issues with supports half the time but that's another problem)
But for about the last 2 weeks I've been having issues with getting filament to extrude properly.

I'm using Repetier and I can heat up the nozzle I've tried anywhere from 190 to 210 on 3 different spools of filament but when I try and just manually push filament through I get a tiny bit and then the gear?/cog? inside rips apart my filament and clogs it self.
I've pulled things apart cleaned out the nozzle and the gears(which don't seem to be damaged) and put it back together and I've messed with the tightness on the screw that adjusts the filament clamp.
but it still happens every time.
And it does push a little filament through and then it just stops, it's like it gets to certain point on the gear and it stops feeding.

I have no idea how to fix this at this point, and help will be appreciated.
